In conjunction with the SCH Case Management System, uses the patient focused care process (assess, plan, implement and evaluate) for Women Children’s Center patients across the continuum of care and ensures the implementation of a comprehensive, collaborative plan of care; ensures the effective, efficient and appropriate use of resources and tracks and assists to monitor patient outcomes. Patients cared for include children from early gestation through adolescence, childbearing women and women with gynecologic diagnoses. Implements and abides by Customer Service Standards.
