Case Managers serve as the care coordinators for patients, care givers, and families. Their functions include concurrent and retrospective review to assure appropriate utilization, assessment and coordination of discharge options. They are required to follow specific criteria sets to assure compliance with all QI/UN/DCP regulations. They coordinate safe discharge plans between the health care delivery team, patients, families, insurers and continuing care agencies. EDUCATION/EXPERIENCE: Registered Nurse - Bachelors degree desired. Excellent clinical skills in acute hospital setting. Experience with DCP/UM desired. Excellent communication and interpersonal skills required. Current Massachusetts RN Licensure required.
